Job Summary
Our Nurses work in collaborative environments with strong
interdisciplinary teams, focused on a patient centered care model
in a richly diverse work space. Under the direction and general
supervision of the Nurse Manager, the registered nurse utilizes
evidence based practices in accordance with the State Nurse
Practice Act, policies and procedures of the hospital and as
directed by the medical staff. The registered nurse who through
knowledge and ability uses the nursing process to develop a plan of
care which includes standards of practice to meet the physical,
emotional spiritual, cultural and educational needs of the patient
and family. -

This Registered Nurse on Inpatient Wound Ostomy Continence Nurse
(WOCN) Team providing wound, ostomy and continence nursing care,
consultation, education and mentorship for hospital based service.
Practice involves prevention and treatment of pressure injury and
other wound types, ostomy/fistula care and patient education, care
for patients with surgical, traumatic, lower extremity, lymphedema
and other wound types. -Performs wound assessment and treatment
interventions, provides patient/family education, mentors
unit-based nurses, supports standardized skin injury prevention
protocol and performs surveillance and monitors outcome metrics. -
-Collaborates with intra-professional teams. - Day shift, rotating
weekends. -Qualifications -
BSN Required. Acute care and wound care experience required.
Graduate of a WOCNCB accredited education program with current
Wound / Ostomy /Continence certification preferred. Exceptions to
the educational requirement will be evaluated on an individual
basis. A strong candidate in the process of completing the
educational requirement will be considered. Weekend coverage
required every 4 - 6 -weeks.
- - - - - - - - - Provides direct care to patients with complex
acute and chronic wounds, urinary and fecal diversions, stoma care,
fistulas, percutaneous tubes, pressure ulcers, lower extremity
wounds, surgical wounds, incontinence/moisture associated dermal
conditions and others.

- - - - - - - - - Provides collaboration and consultation to the
health care team in developing and implementing plans used in the
identification and management of patients with potential or actual
alteration in skin integrity and stoma /fistula care.

- - - - - - - - - Provides appropriate education to patients,
families, caregivers, and staff regarding wound and skin care,
ostomy care, percutaneous tubes, and draining wound/fistulae
management.

- - - - - - - - - Evaluates patient response to treatment in the
progress of wound healing, ostomy self-care skills training and
other outcomes; and adjusts plan of care as indicated.

- - - - - - - - - Uses protocols and evidence-based guidelines for
appropriate and cost-effective use of therapies, products and
equipment to prevent and treat skin/wound conditions and ostomy
/fistula management.

- - - - - - - - - Actively participates in orientation, training
and champion programs, Quality improvement activities, data
analytics and policy /procedure development. -

- - - - - - - - - Provides accurate and timely written
documentation of care in the electronic medical record.

- - - - - - - - - Joins others on the Inpatient WOCN Team at
Hartford Hospital.
