Job Description:
A company is looking for a Telephonic Nurse Care Associate to
provide telephonic interventions for patients with chronic
conditions.
Key Responsibilities:
Complete patient interventions and follow up on biometrics
readings
Engage patients using Motivational Interviewing techniques to
promote adherence to care plans
Document patient interactions accurately and maintain clinical
charts
Required Qualifications, Training, and Education:
Nursing degree from an accredited college or university or relevant
experience in lieu of a degree
Minimum of 5 years of broad clinical experience
At least 3 years of experience in home care, care coordination, or
telehealth
LPN/LVN licensure in the appropriate state(s), active and in good
standing
Strong computer skills for navigating multiple programs and
troubleshooting basic issues
